• Go to appointment scheduling at the top of the page.
• You must choose the date first and then chooseÌýFluÌýShotÌýClinic.ÌýAvailable times will appear.ÌýÌý

Prior to your appointment:

• Upload a copy of your insurance within the health portal or email an image toÌýrwelp@gustavus.edu
• Within 24 hours of your Appointment - CompleteÌýImmunizationÌýConsent Form, Health Insurance Form and Notice of Privacy Form withinÌý
