How to sign up:Ìý

• Go to theÌýand sign in.
• Go to appointment scheduling at the top of the page.
• You must choose the date first and then chooseÌýFluÌýShotÌýClinic or COVID-19ÌýVaccine. Available times will appear.ÌýÌý
• If you want bothÌývaccines, you only need to schedule for one time slot! Just let us know when you come to your appointment that you would like both.

Prior to your appointment:

• Upload a copy of your insurance within the health portal or email an image toÌýrwelp@gustavus.edu
• Within 24 hours of your Appointment - Complete Immunization Consent Form, Health Insurance Form and Notice of Privacy Form withinÌý
