• Go to theÌýÌýand sign in.
• Go to appointment scheduling on the top of the page.
• You must choose the date firstÌý (10/11/22 or 10/12/22) and then choose Flu Shot Clinic. Available times will appear.ÌýÌý

Prior to your appointment:

• Complete the Vaccine Consent Form within the health portal.ÌýÌý
• Upload a copy of your insurance within the health portal or email an image toÌýrwelp@gustavus.edu.
